How to Get the Account Opening Form (CBE)

The CBE (Customer Base Entry) form can be obtained through the following methods:

1. Online registration via M-STOCK application (recommended — paperless process).
2. Walk-in at our nearest branch office. Bring your original KTP and NPWP.
3. Download the form from our official website and fill it in manually.

Notes

• The CBE form must be filled in completely without any blank fields.
• Corrections must be initialed by the applicant.
